Lott Physical Therapy provides home physical and occupational therapy services throughout Freestone/Limestone/Leon Counties. This is a contracted service through the following home health agencies:

Physical Therapy is initiated after an order is received from a physician, and performed through one of the above home health agencies. Physical and Occupational Therapy within the home will include an evaluation of the patient's status, as well as a home safety assessment. Depending on the patient's diagnosis treatment sessions may include:
• Instruction in adaptive equipment
• Gait training with or without an assistive device
• Instruction in exercises
• Activities of daily living

Home Health Care Services from Lott Physical Therapy
Home Health Care under Medicare Guidelines:
• Patient is eligible for Medicare coverage.
• Patient's medical condition requires a skilled service including one of the following: Physical Therapy, Occupational Therapy, Speech Therapy, or Nursing.
• Patient is homebound; it is difficult or presents a safety risk for the patient to get out of his or her home.
• Patient is under the care of an attending physician who is seeing the patient on a regular basis.
• Patient wants the service and agrees to it in writing.
